Pre-departure Checklist
First, please print out and review the Pre-departure Checklist (Download PDF). It contains critical passport, Visa, and health information, as well as due dates for all information. All forms and a copy of your passport's ID page must be printed and faxed to Elizabeth Shaw at 617-373-8929 before June 1 unless otherwise noted.
